To register, please complete this form and submit it to the Village Office, 215 E. 4 Mile Rd., Racine, 53402 along with the $10.00 registration fee. A separate form and fee are required for each rental property. Please note your registration is effective for a period of two years and then it must be renewed. There is no fee required to renew your registration. If there is a change in contact information during the registration period, the owner must notify the Village within 15 days of the change.
